Emptying the breasts and expressing breastmilk

It is good to learn the correct technique of breast massage and milk expression already in the maternity hospital, in case you happen to need it.
Body

Immediately after birth, the breasts begin to fill with milk in a small, but sufficient amount for the baby. If the baby is properly positioned on the breast, is breastfed responsively and efficiently empties the breasts, ideal conditions of supply and demand will be created (the amount of milk corresponds to the baby's needs). Milk expression will not be needed in that case. Breasts can sometimes become hard, tense and painful. All babies are born with the rooting, sucking and swallowing reflex, but some babies are "stronger", so they suck more efficiently immediately after birth, and some need help.

Expression is required:

  • if the baby does not suckle enough milk from the breasts in the first days because he/she was born prematurely, with low weight, or has jaundice
  • when you and your baby are separated due to illness
  • in case of plugged milk ducts, damaged nipples, inflamed breasts
  • when you want to encourage the production of more milk
  • when you plan to be away from home for a longer period of time or have returned to work, so that the person caring for the baby can feed him/her your milk
  • when you want to donate your milk to a breastmilk bank

Depending on the needs, you can give the expressed milk to your child, store it in the refrigerator or freezer for later meals, transport it to the child (if the baby is in hospital or you are separated for other reasons). Follow the recommendations for keeping and storing breastmilk!

Hand expression of milk

Preparation:

  • Wash your hands with soap and water
  • Massage will stimulate the milk ejection reflex and facilitate flow. Gently massage the breast: start from the base and move towards the nipple; do not forget the underside of the breast. Repeat several times, in order not to skip any part. The massage should be gentle, with light pressure, or tapping with the fingertips (as on piano keys).
  • Warm compresses before the massage can also facilitate milk flow.
  • Place the milk container (preferably a bowl with a wide opening) under the breast, between the knees or on a low table, so that both hands are free.
  • Prepare a towel to wipe off any drops that "escape" or slippery, wet fingers.

Expression:

  • You can hold the breast with your other hand, if it is large or heavy.
  • Place your thumb and other fingers opposite each other, away from your nipple.
  • Using your thumb and first two fingers, press first inward, toward the center of the breast; expression must not be painful.
  • Then gently squeeze, in rhythmic movements, as if you wanted to join the thumb and other fingers; this way, you compress the ducts and the milk starts flowing.
  • The milk first flows out in drops, and then in a jet, when the milk ejection reflex kicks in.
  • Rotating your thumb and forefinger around the areola will allow you to express milk from all parts of the breast.
  • Empty the breast for 3-5 minutes, until the flow begins to decrease, then switch to the other breast. Repeat the whole procedure once more, so that the breast emptying lasts a total of 20-30 minutes.
  • Breast emptying can be tiring; take a break and drink some water.
  • Avoid rubbing the skin roughly and squeezing the nipple; this will lead to swelling and prevent milk flow.

Expressing milk with a pump

  • Put warm compresses on your breasts a few minutes before pumping.
  • Gently massage both breasts.
  • Express some milk from both breasts.
  • Press the pump flange and the silicone massage shield firmly on the breast.
  • Slowly push the pump handle down, hold for a few seconds, then let go, mimicking the rhythm of sucking.

In addition to manual pumps, there are various electrical ones. They mimic manual pumping, making the process much easier because the pump works for you. A disadvantage is their high price. Some moms find it easier to hand express their breasts, and some to use a pump. If you intend to buy a pump, consult professionals about the choice, and consider whether you need one at all.

The amount of milk

Some women find it easy to express, and some need time to learn the correct technique. The amount of milk you will get by expressing on a number of factors: the breasts, the baby's age, time of the last breastfeed, frequency of emptying your breasts. If you get only a small amount or none at all, seek help to make sure you are doing it correctly.

Expressing will be easier and more successful if you make yourself comfortable and provide privacy. Prepare a glass of water and set aside enough time, especially if you are a beginner in this.
